{"id":200,"date":"2018-10-22T18:40:45","date_gmt":"2018-10-22T18:40:45","guid":{"rendered":"https:\/\/www.proacls.com\/wiki\/?p=200"},"modified":"2018-11-11T02:13:45","modified_gmt":"2018-11-11T02:13:45","slug":"procainamide","status":"publish","type":"post","link":"https:\/\/www.proacls.com\/wiki\/acls-pharmacology\/procainamide\/","title":{"rendered":"Procainamide Pharmacology"},"content":{"rendered":"<h2><iframe loading=\"lazy\" src=\"https:\/\/www.youtube.com\/embed\/ptUzWICh93I\" width=\"560\" height=\"315\" frameborder=\"0\" allowfullscreen=\"allowfullscreen\" data-mce-fragment=\"1\"><\/iframe><\/h2>\n<h2>The Effects of Procainamide:<\/h2>\n<p>Procainamide is effective in slowing the conduction in the atria, ventricles, and bundle of His Purkinje system. \u00a0It does this by:<\/p>\n<ul>\n<li>Prolonging the PR intervals and QT intervals<\/li>\n<li>Slowing the refractory periods of the atria and ventricles<\/li>\n<li>Slowing the refractory period of the atrioventricular node<\/li>\n<\/ul>\n<h2>Procainamide Indications for Use:<\/h2>\n<p>Procainamide has been effective for the treatment of <a href=\"https:\/\/www.proacls.com\/wiki\/ekg-rhythms\/tachycardia-ecg-ekg\/\">supraventricular tachycardia<\/a> that returns after vagal maneuvers and <a href=\"https:\/\/www.proacls.com\/wiki\/acls-pharmacology\/adenosine\/\">adenosine<\/a> were ineffective.<\/p>\n<p>It helps treat:<\/p>\n<ul>\n<li>Stable wide complex tachycardia of uncertain origin<\/li>\n<li>Stable monomorphic ventricular tachycardia with normal QT interval<\/li>\n<li>Atrial fibrillation with a rapid ventricular rate response in patients with Wolff-Parkinson-White (WPW) syndrome and recurrent <a href=\"https:\/\/www.proacls.com\/wiki\/ekg-rhythms\/ventricular-fibrillation\/\">ventricular fibrillation<\/a> or pulseless ventricular tachycardia<\/li>\n<\/ul>\n<h2>Procainamide Precautions &amp; Contraindications:<\/h2>\n<p>There are some contraindications for the use of procainamide. \u00a0The ACLS team should be aware of any known sensitivity to procainamide or similar medications for the patient.<\/p>\n<p>Digitalis toxicity may complicate already existing atrioventricular conduction depression. Other contraindications would include third degree heart block or pre-existing prolongation of the QRS complex and QT intervals. Procainamide should be avoided in patients with prolonged QT intervals and associated CHF.<\/p>\n<h2>Adult Dosage for Procainamide:<\/h2>\n<p>The use of procainamide is limited in ACLS for <a href=\"https:\/\/www.proacls.com\/wiki\/acls-algorithms\/adult-cpr\/\">cardiac arrest<\/a>, due to its requirement of slow infusion and its somewhat unknown effectiveness.<\/p>\n<p>If used for recurrent ventricular fibrillation and pulseless ventricular tachycardia, 20 mg\/min should be given via IV infusion to a total dose of 17 mg\/kg. \u00a0For supraventricular tachycardia, atrial fibrillation, and wide complex tachycardias of uncertain origin, procainamide should be administered at 20 mg\/min via IV infusion to a total of 17 mg\/kg.
