Lidocaine can calm erratic and uncoordinated electro-myocardial activity.<\/p>\n<h2>Indications for Use of Lidocaine:<\/h2>\n<p>Due to lidocaine\u2019s antiarrhythmic properties, its primary use is for:<\/p>\n<ul>\n<li>Cardiac arrest from <a href=\"https:\/\/www.proacls.com\/wiki\/ekg-rhythms\/ventricular-fibrillation\/\">Ventricular Fibrillation<\/a><\/li>\n<li>Pulseless <a href=\"https:\/\/www.proacls.com\/wiki\/ekg-rhythms\/wide-complex-ventricular-tachycardia\/\">ventricular tachycardia<\/a><\/li>\n<\/ul>\n<p>Lidocaine is also effective in treating:<\/p>\n<ul>\n<li>Stable monomorphic ventricular tachycardia with preserved ventricular function<\/li>\n<li>Stable polymorphic ventricular tachycardia with normal baseline<\/li>\n<li>QT interval and preserved lower ventricular function when ischemia is treated and electrolyte balance is corrected<\/li>\n<li>Stable polymorphic ventricular tachycardia with baseline and QT interval prolongation when Torsades de pointes is suspected<\/li>\n<\/ul>\n<h2>Lidocaine Precautions &amp; Contraindications:<\/h2>\n<p>Lidocaine should not be used as a prophylactic treatment in acute myocardial infarction.<\/p>\n<p>The maintenance dose should be reduced in the presence of impaired liver function or lower ventricular dysfunction. \u00a0Infusion should be discontinued immediately if signs of toxicity develop.<\/p>\n<p>Lidocaine would be contraindicated if there is a known hypersensitivity to lidocaine or derivatives like xylocaine, novocaine, etc.<\/p>\n<p>Lidocaine is also contraindicated in sinus bradycardia and atrioventricular blocks.<\/p>\n<h2>Adult Dosage for Lidocaine:<\/h2>\n<p>Dosage for cardiac arrest from ventricular fibrillation or pulseless ventricular tachycardia:<\/p>\n<ul>\n<li>Initial dose is 1 to 1.5 mg\/kg IV or IO<\/li>\n<li>Can also be delivered via endotracheal tube<\/li>\n<\/ul>\n<p>Dosage for refractory ventricular fibrillation:<\/p>\n<ul>\n<li>An additional .5 to .75 mg\/kg may be given via IV push<\/li>\n<li>This can be repeated in 5 to 10 minutes to a maximum of 3 doses or a total of 3 mg\/kg<\/li>\n<\/ul>\n<p>Dosage used for perfusing arrhythmias, like stable ventricular tachycardia, wide-complex tachycardia, or an uncertain type or significant ectopy:<\/p>\n<ul>\n<li>Doses range from .5 to .75 mg\/kg and up to 1 to 1.5 mg\/kg<\/li>\n<li>This can also be repeated at .5 to .75 mg\/kg every 5 to 10 minutes to a maximum total dose of 3 mg\/kg<\/li>\n<li>For a maintenance infusion, 1 to 4 mg per minute should be given, which is equal to 30- 50 mcg\/kg per minute<\/li>\n<li>A microdrip infusion set is needed in order to deliver the appropriate dose<\/li>\n<\/ul>\n<h3>A common calculation for mixing a lidocaine drip is:<\/h3>\n<p style=\"text-align: center;\">(IV Bag Amount in mL) \u00a0X (Dose Ordered (mg\/min)) \u00a0X (Drip Set (gtts\/mL)<br \/>\n<em>Divided by<\/em><br \/>\n(Drug on Hand in mg)<br \/>\n= Correct drops per minute<\/p>\n<p>&nbsp;<\/p>\n","protected":false},"excerpt":{"rendered":"<p>The Effects of Lidocaine: Lidocaine causes negative inotropic effects and antiarrhythmic actions in the heart that weaken the force of muscular contractions.
